Health information we create or receive in connection with providing laboratory testing services — including test orders, specimen results, and related clinical records — is governed by our Notice of Privacy Practices, which describes our obligations under the Health Insurance Portability and Accountability Act ("HIPAA"). Information You Provide to Us
The information we collect on or through our Website may include:
- Contact information, such as your name and email address, when you submit our contact form
- Career application information, such as your name, email, phone number, position of interest, resume/CV file, and any cover message you choose to submit
- Order information when you purchase a self-pay test through our Shop, such as your name and email address; payment card details are collected and processed directly by our payment processor, Stripe — we do not receive or store your full card number
- Any other information you choose to send us, for example in the free-text fields of our forms
Information Collected Automatically
As you navigate through our Website, our hosting provider and standard web analytics tools may automatically collect certain technical information about your equipment, browsing actions, and patterns, such as your IP address, browser type, referring/exit pages, and the dates and times of your visits. We do not currently use this information to identify you individually.
Information From Third Parties
When you complete a purchase, our payment processor (Stripe) provides us with confirmation of your order and a limited set of transaction details (such as the last four digits of your card and the billing name/email) so that we can fulfill your order and provide customer support.
How We Use Your Information
We use information that we collect about you or that you provide to us to:
- Respond to inquiries submitted through our contact form
- Review and respond to career applications
- Process, fulfill, and provide support for self-pay test orders placed through our Shop
