Direct-to-Consumer Testing Informed Consent

Purpose of This Consent
Some tests offered through the Matrigene Laboratories Shop can be ordered directly by patients without a physician referral ("direct-to-consumer" or "self-pay" testing). Please read this page carefully before placing an order — by completing a self-pay order, you acknowledge that you have read, understood, and agree to the terms described here.
Voluntary Participation
Self-pay testing is entirely voluntary. By ordering a test through our Shop, you are requesting laboratory testing on your own behalf, at your own initiative, without a physician's order. You may decline to proceed with testing at any time before your specimen is collected.
What to Expect
After placing an order, you will typically visit our lab or arrange for at-home specimen collection, depending on the test. Your specimen is processed in our CLIA-certified laboratory, and results are made available to you as described in our Patients resources.
Benefits and Limitations of Testing
Laboratory testing can provide useful information about your health and help identify conditions that may require follow-up care. However, no laboratory test is perfect. Results can be affected by factors such as timing, sample quality, medications, or underlying conditions, and no test eliminates the possibility of a false positive or false negative result. A single test result is one piece of information about your health, not a complete diagnosis on its own, and some results may require confirmatory testing or follow-up care with a healthcare provider.
Result Delivery & Interpretation
We strongly recommend reviewing your results with a licensed healthcare provider, particularly if a result falls outside the normal reference range. Matrigene Laboratories does not provide medical diagnosis, treatment recommendations, or emergency medical care, and our staff cannot interpret your results for you or tell you what they mean for your specific health situation.
Not for Emergency Use
Self-pay testing is not appropriate for, and should never be used to evaluate, a medical emergency. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately rather than waiting for test results.
No Physician-Patient Relationship
Ordering a self-pay test does not create a physician-patient relationship with Matrigene Laboratories. We are a clinical laboratory, not a medical practice, and our role is limited to specimen collection, processing, and reporting of results.
Specimen Collection Risks
Most tests require a blood, urine, or swab specimen. Blood draws carry minor, well-recognized risks such as bruising, discomfort, or, rarely, lightheadedness or infection at the collection site. Our phlebotomists are trained to collect specimens safely, and you should tell your phlebotomist about any concerns (such as a history of fainting) before your draw.
Costs & Refunds
Self-pay tests are billed at the time of order at the price displayed at checkout. Because specimen collection and processing involve real costs, orders that have already been collected or processed are generally non-refundable; see our Terms & Conditions for details.
Right to Withdraw
You may withdraw your consent to testing at any time before your specimen is collected by contacting us. Once a specimen has been collected and submitted for processing, we may be unable to cancel the test.
Privacy & Confidentiality
Your health information is handled in accordance with our Notice of Privacy Practices and Privacy Policy.
Consent to Testing
By placing an order through our Shop, you acknowledge that you are at least 18 years of age (or are ordering on behalf of a minor as their parent or legal guardian), that you have read and understood this Informed Consent, that testing is voluntary, and that you agree to have your specimen collected and tested as described above.
